Please complete the required information highlighted, and follow the prompts to return the completed signed document. After we receive this document, you will receive an e-mail with your receipt. Your membership will be immediately activated and ready to utilize. Please call the spa at 517-323-4331 to schedule appointments or submit a list of eligible employees who may use the services.
